You have obtained proper consent from the individual being tested
Clients agree to comply with the Fair Credit Reporting ACT (FCRA) and all applicable laws

All specimens are collected following industry standards and applicable regulations
VeriQuest Labs does not guarantee outcome or results
Test results reflect only the condition of the specimen at the time of collection
Factors such as improper collection or insufficient specimen may result in canceled or recollected tests

Fees & Payments

All payments must be made in full before services are rendered
All fees are non-refundable once services have been performed
Additional fees may apply for recollections, expedited services, or incomplete submission
Square is used for payments (online or in office) No Cash payments

VeriQuest Labs is committed to protecting client and donor privacy. All information is handled in accordance with applicable privacy laws, including HIPPA. Results are released only to authorized parties as designated by client or donor.

Confidentiality & Privacy

To the fullest extent permitted by law, VeriQuest Labs shall not be held liable for:

Indirect, incidental, or consequential damages
Decisions made by Clients or third parties based on test results
Misuse or misinterpretation of results
Our total liability shall not exceed the amount paid for the specific service provided.

Limitation of Liability

VeriQuest Labs Service Agreement This Service Agreement is entered into between VeriQuest Labs (”VQL “we,” us,” or “) and the individual or entity requesting services (“Client,” “you,” or “your”). By scheduling, ordering, or receiving services from VeriQuest Labs, you agree to the terms outlined below.

Turnaround Times

Estimated turnaround times are provided as a courtesy and are not guaranteed. Delays may occur due to laboratory processing or verification requirements or circumstances beyond our control. Typically, you will receive results within 24-48 hours

Put pets away
Secure all weapons (must be put away)
No drugs or Alcohol should be present
Provide a clean area for collection process to take place
Failure to follow required procedures will result in termination of the collection, and NO REFUND will be issued
